Vision Plan

2010 Vision Plan Document

Participation in the Vision Plan is determined by the Medical Insurance Plan. If the employee and/or eligible dependents are enrolled in the Medical Insurance Plan, then coverage in the Vision Plan is included. There is no premium charged for the Vision Plan.

CIGNA is the provider of record for the vision plan.

Co pays and allowable expenses are subject to plan guidelines.

Provider reimbursement is determined by participation in the CIGNA provider network.

Participating providers can be accessed by utilizing the myCIGNA.com website located on the right side of this website.
